  • Describe the inaccuracies you found in the note, and explain why these are inaccuracies.
  • Explain how the identified inaccuracies should be corrected.
  • Explain the importance of accurately interpreting medical terminology. Consider the impact on both patient care and health documentation.

Respond to at least two of your peers by pointing out any inaccuracies they did not see or providing suggestions on how to interpret the inaccuracies. In addition, discuss commonalities or differences in the ways your peers explained the importance of accurately interpreting medical terminology. What insight can you provide your peers? Did your peers identify reasons that you found particularly insightful?

Note: You must submit your initial post before you can see your peers’ posts.

HCM 205 Discussion

Rubric

The discussions in this course might be new and very different from discussions you have had in other courses. This discussion offers you the opportunity to
express your own thoughts, ask questions for clarification, and gain insight from your classmates’ responses and instructor’s guidance. This discussion format
should not be viewed as a short paper with citations. APA formatting, including the use of citations, is not required.

Imagine yourself sitting around a conference table with your instructor and classmates. This discussion board will be similar. It is a place to have lively
conversations on the course content with your instructor and peers. These discussions will help you internalize the course content and become more
comfortable with it. You should provide thoughtful responses using the course content to support your statements. Although these discussions are informal, be
aware of how you are presenting yourself professionally. Do not use slang and be aware of your spelling.

You are required to post one initial post and to follow up with at least two response posts for each discussion board assignment. You will need to submit your
initial post before you can see your peers’ posts.

For your initial post (1), you must do the following:

 In Module One, complete the initial post by Thursday at 11:59 p.m.
Eastern Time.

 In Modules Two through Eight, complete the initial post by Thursday at
11:59 p.m. of your local time zone.

 Take into consideration material such as course content and other
discussion forums from the current module and previous modules,
when appropriate.

For your response posts (2), you must do the following:

 Reply to at least two different classmates outside of your own initial
post thread.

 In Module One, complete the two response posts by Sunday at
11:59 p.m. Eastern Time.

 In Modules Two through Eight, complete the response posts by Sunday
at 11:59 p.m. of your local time zone.

 Demonstrate more depth and thought than simply stating “I agree” or
“You are wrong.” Guidance is provided for you in each discussion prompt.

History & Physical Exam

CHIEF COMPLAINT: Difficulty breathing

HISTORY OF PRESENT ILLNESS: This is an 8-month-old white male seen by Dr. White at the Lofty Pines Health Center for a viral-type syndrome. The mother states that the baby had a temperature over the weekend with it being 134 on Saturday night and 103 last night. The baby has progressively gotten more worked up, more short of breath and has had more problems with congestion. The baby was admitted for observation and evaluation of his upper resporitory and lower respiratory disease. This is the first hospitalization for this child.

PAST MEDICAL HISTORY: This baby only has one other hospitalization besides that of birth and that was for admission to the hospital for respiratory distress syndrome and was taken care of by Dr. Smith in the neonatal intensive care unit. He only stayed less than a week and did quite well and there was no need for follow up. The mother states that the patient was born three weeks early and the doctors felt that this was secondary to her having a kidney stone. The child had a normal, spontaneous, vaginal birth. He was 19 inches at birth and weighed 6 pounds 15 ounces.

GENERAL: This is an 8-month-old looking his stated age with respiratory problems with a rapid rate and rather annoyed with a stuffed-up nose.

TEMPERATURE: 101.3 PULSE: 158 RESPIRATIONS: 62

EENT: Extraocular movements are full. Pupils are equal, round, and reactive to light and accommodation – red reflexes within normal limits. Nose – stuffed up with clear rhanirea. Mouth – within normal limits.

CHEST: No inflammation of the bronchioles.

HEART: Regular rhythm. Without murmur, rub, or gallop that I can detect, but the patient indeed had a tachycardia at the time of examination. The patient has low blood pressure.

ABDOMEN: Within normal limits.

LAB DATA: Complete Blood Count revealed: White count was 10.7, hemeglabon was 12.2, hematocrit was 37.7, 13 segmented cells, 77 lymphocytes, 10 monocytes. Uranalisis was within normal limits except for trace blood on dipstick; doubt whether that is significant. Chest x-ray was within normal limits.

IMPRESSION: 1. Bronchiolitis 2. History of premature delivery with one hospitalization for lung immaturity at time of birth. 3. Anisocoria

Plan: Patient will need forced fluids and close observation. Provide patient decongestant. Provide patient with COX-2 inhibitor.
