Vulnerable Population Summary and Proposed Program
The first of your two written assignments for the course will provide a beginning framework that you will utilize in the development of your Final Project: a proposal for a community-based program in your area. For this first written assignment, you will select one of the vulnerable groups identified in the text that will serve as your target population of interest throughout the duration of your next written assignment and Final Project.  Choose one among the following groups from Chapter 1:

Once you have selected a group of interest, write a three- to- four page paper that covers the following:

  1. Describe the impact that at least two of the factors below have on the vulnerability of your chosen group:

  3. Analyze the intersection of social, political, and economic factors affecting vulnerability (must address all three factors).
  4. Draft the design of a model program. Provide a two- to- three paragraph statement that introduces your proposed community program. This section is tentative and might change as you conduct more research. At a minimum, however, items to address should include:

Identify the health needs of the group and select a proposed continuum of care level focus (preventive, treatment, or long-term care) based on the group’s stated risk factors and needs.Discuss your justification for the proposed level with supportive research/evidence.A description of one- to- two proposed services your program will include.

Ashford 4: – Week 3 – Assignment

Assessment of Community-Level Barriers
For the second written assignment of the course, you will continue in the design of your proposed model program by demonstrating your understanding of your selected population’s challenges, which negatively impact this group’s health and well-being. Based on this week’s research, conduct an assessment of the barriers, limitations, and other distinguishing features, as they exist within your community. Your paper, at a minimum, must contain the following elements:

  1. A description of at least three critical barriers that impact the health and well-being of a chosen group; one must be a micro-level (individual) barrier that is financial, one must a macro-level (community/state) barrier that relates to access and funding for care, and  the third barrier may be one of your choosing.
  2. An analysis of the regulatory, legal, ethical, and accreditation requirements/issues that relate to these barriers.
  3. Identify and describe at least one proposed solution for each barrier. Your solution for the micro barrier must include an analysis of various potential funding options (both independent and integrated). Your solution for the macro barrier must include an analysis of financing resources for health care.

Your assignment should be a minimum of  three- to- four pages in length (excluding title and reference pages), and should include a minimum of three scholarly sources cited according to APA guidelines as outlined in the Ashford Writing Center.

,  the struggles and barriers of military service members are highlighted. Although many resources are available to the military, and most are provided at no cost, the suicide rate is enormous.  Please answer the subsequent questions:

  • Why are some hesitant to consult available and free resources?
  • What are your suggestions on how these barriers can be reduced or eliminated?

This week’s first discussion forum will focus on individuals with mental conditions.
  Our health, or the absence of such, can be very expensive. Many vulnerable populations simply do not have private funds available to pay for their care.  Relating your research to individuals with mental conditions, select a mental illness of your choice and answer the following questions:

  • What public financing resources are available to individuals with this particular mental condition?
  • What are the eligibility requirements and restrictions to receive these funds?
  • What happens if eligibility standards/requirements are not met by those with this illness?
  • Are there any alternative care options specific to this group that might be available?

This week’s first discussion forum will focus on persons diagnosed with HIV/AIDS. 
  Select an existing program or service delivery model in your community or surrounding environment (e.g., free-standing clinic, specialized treatment center) that provides services to those impacted by HIV/AIDS. Describe both, direct as well as indirect, costs associated with the program and how the program is evaluated (cost-benefit or cost-effective analysis). Chapter 7 of the course text defines these terms in more detail. Your initial contribution should be 250 to 300 words in length. Your research and claims must be supported by a minimum of two scholarly sources beyond your course text.  Use proper APA formatting for in-text citations and references as outlined in the Ashford Writing Center. This week’s second discussion forum will focus on indigent and homeless people. As highlighted in Chapter 9 of your course text, three research methods are available (i.e., descriptive, analytic, and evaluative). Discuss these methods and give one example of how each would be used when conducting research using homeless individuals as subjects. Analyze possible regulatory, accreditation, ethical, and legal issues and challenges with respect to your research.   Your initial contribution should be 250 to 300 words in length. Your research and claims must be supported by a minimum of two scholarly sources beyond your course text.  Use proper APA formatting for in-text citations and references as outlined in the Ashford Writing Center. Variations in Approaches to Care

This week’s first discussion forum will focus on immigrants and refugees.
Describe the basic premise of both the community and market-oriented approach to health care. To what extent is each multidisciplinary? How might each approach help and/or hinder health care access for immigrants and refugees? What are ways in which these approaches can promote a culturally inclusive health care experience for this group? Include a brief analysis of the role that health delivery models can play alongside these approaches in the provision of culturally competent care. Your initial contribution should be 250 to 300 words in length. Your research and claims must be supported by a minimum of two scholarly sources beyond your course text.  Use proper APA formatting for  in-text citations and references as outlined in the Ashford Writing Center. For this final forum, describe a multidisciplinary approach for a vulnerable population that we have not covered in class. Imagine you are a policymaker for your city or town. Based on what you know, and what you have learned in this course, what would you modify or improve to make a positive change in the life of a vulnerable group we have not covered in class? Would you consult the vulnerable group when developing programs or policies? Why or why not? Which policy would you create to improve health care services accessibility, cost, and quality related to your selected group? Your initial contribution should be 250 to 300 words in length. Your research and claims must be supported by a minimum of two scholarly sources beyond your course text.  Use proper APA formatting for  in-text citations and references as outlined in the Ashford Writing Center. Guided Response: Review several of your classmates’ posts.  Provide a substantive response (minimum of 100 words) to at least two of your peers. What new things did you learn about another vulnerable population from your colleagues?

       

Final Project Imagine you are a policymaker or city council member and are actively involved in improving the lives of the vulnerable.  Via the first two written assignments, you have already selected one vulnerable population, which is in need of a new program or service within your community. You will research and design a presentation (APA style Paper or PowerPoint Presentation), which you will present at the next city meeting to advocate for your special population group.
  The Final Project should demonstrate an understanding of the reading assignments, class discussions, as well as your own research and application of new knowledge.  It must contain the following elements:

  1. A description of why this population group is vulnerable and in desperate need for help within your community.
  2. Identification of the health service needs of this special population.
  3. In order to support your claims and opinions, cite statistical data obtained from the county health department, state health department, and/or other organizations and agencies.

Design a model program or service, which is needed to improve the lives of your chosen population within your community. Remember to design a new program not currently existent within your community.  In your design, include the following:

  1. A description of the continuum of care level (preventive, treatment, or long-term care) your program or service fall into and how it meets the needs of your selected group.  
  2. An analysis of the delivery modes of health services to be included.
  3. A discussion regarding the multidisciplinary approach that is incorporated into the program.
  4. An analysis of the role of legal and ethical requirements in the design of the program.

Be detailed and convincing in describing the above elements, as you are attempting to win as much support from the attending members as possible.
  Identify a minimum of two community agencies/organizations you would like to partner with in some way(s) in order to implement this program/service.  Explain why you would like to partner with these organizations and how can they help or benefit you with the implementation of your program/service.   Analyze the current funding sources for provision of health services to this special population. Discuss the use of integrated funding. Based on your research, identify a minimum of two funding sources that may potentially provide monetary sponsorship for your program or service.  Explain why these entities would be likely funding sources. For your presentation, you may choose between the following formats:

  1. An APA formatted Final Paper
  2. A PowerPoint Presentation.

Writing the Final Paper The Final Paper:

  1. Must be eight to ten double-spaced pages in length, and formatted according to APA style as outlined in the Ashford Writing Center.
  2. Must include a title page with the following:

  4. Must begin with an introductory paragraph that has a succinct thesis statement.
  5. Must address the topic of the paper with critical thought.
  6. Must end with a conclusion that reaffirms your thesis.

  8. Must document all sources in APA style, as outlined in the Ashford Writing Center.
  9. Must include a separate reference page, formatted according to APA style as outlined in the Ashford Writing Center.

Creating the PowerPoint Presentation The PowerPoint:

  1. Must be 18 to 20 slides, excluding title and reference slides.
  2. Must include a title slide with the following:
